Occupational Therapist

Hope Center is actively seeking Occupational Therapists to work in the Therapy Department at our Skilled Nursing Facility in Bronx, NY.

OT Duties Include:

  • OT's Evaluate Residents & develop a treatment plan that will meet their needs.
  • Choose activities that will help the Residents to carry out their daily works independently.
  • Monitor Residents as they carry out the task given to them.
  • OT's Observe & assess Residents' progress and prepare detailed reports accordingly.
  • Help in rehabilitating Residents disabled as a result of injury, illness or other problems.
  • Maintain Resident confidence by monitoring confidential information processing.
  • Maintain documentation of Resident care services by auditing records.
  • Maintains a cooperative relationship among healthcare teams & contributes to team effort.

 

OT Requirements Include:

  • Graduate of an accredited Occupational Therapy Program
  • Current New York State OT License
  • Experience in long term care settings preferred

Hope Center is looking to hire Physical Therapists for the Therapy Department at our Skilled Nursing Facility in Bronx, NY.

Substance Abuse Counselor

Hope Center is actively seeking a Certified Substance Abuse Counselor to work in our Skilled Nursing Home Facility to provide substance abuse treatment services according to Hope Center standards.

 

DUTIES:

  • Conduct individual and group counseling.
  • Maintain resident charts.
  • Develop and oversee Treatment Plans
  • Provide relapse prevention related information/education to residents on a regular basis through small groups, seminars and individual sessions.
  • Develops and updates, as needed, Substance Abuse Treatment Plan.
  • Coordinates resident's treatment with other program departments.
  • Assists residents in understanding the various phases of the treatment process.
  • Performs welcome interview upon admission.
  • Review and distributes residents of program policies, resident's rights and responsibilities.
  • Make facility rounds to ensure the safety, security and integrity of the environment.
  • Monitor resident behavior and compliance with care plan.
  • Cover clinic desk to monitor call bell system as needed.
  • Monitor residents' rooms for compliance with regulations.

 

Qualifications:

  • Bachelor's Degree or, completing
  • College for Bachelors Degree in Human Services or,
  • Be a credentialed CASAC. Counselor
  • Minimum of one year recent experience working with substance abusers, T.C. HIV/AIDS, or in a related field. 
  • Fluent in Spanish is a plus

Ambulette Driver

Hope Center is looking to hire a FT Ambulette Driver to transport patients between their homes and our facility.

 

DUTIES:

  • Transporting residents to & from appointments
  • Transporting family members to visit residents if needed
  • Schedule: Full Time

REQUIREMENTS:

  • Handy man experience, as it would be required
  • Minimum of a CDL Class-C license with passenger endorsements
  • Experience driving Ambulette vehicles / ambulatory services
  • No more than 4 points on license, with NO outstanding violations
  • Organized, and knowledgeable of the 5 boroughs
  • Able to adapt to fast pace environment
  • Eligible to work in the United States
  • Minimum of high school diploma or equivalent
  • Flexible availability is a MUST
  • Need to be dependable, responsible and a team player with problem-solving skills
  • Must have professional references
