Centers Plan for FIDA Care Complete Disclaimers

Centers Plan for FIDA Care Complete (Medicare-Medicaid Plan) is a managed care plan that contracts with both Medicare and the New York State Department of Health (Medicaid) to provide benefits of both programs to Participants through the Fully Integrated Duals Advantage (FIDA) Demonstration.

For information on Centers Plan for FIDA Care Complete, contact Centers Plan for FIDA Care Complete or the New York Enrollment Broker. To enroll, or for other options for your health care, call the New York Enrollment Broker at 1-855-600-FIDA (1-888-329-1541 for TTY users) Monday through Friday, 8:30 am to 8:00 pm, Saturday 10:00 am to 6:00 pm; or visit http://www.nymedicaidchoice.com/.

In general, Participants must use network pharmacies to access their prescription drug benefit, except in non-routine circumstances, and quantity limitations and restrictions may apply.

In general, Participants must receive all routine care from plan providers. Please contact Participant Services or your Care Manager at 1-800-466-2745 for additional information. TTY users please call 1-800-421-1220.

The State of New York has created a participant ombudsman program called the Independent Consumer Advocacy Network (ICAN) to provide Participants free, confidential assistance on any services offered by Centers Plan for FIDA Care Complete. ICAN may be reached toll-free

at 1-844-614-8800 or online at icannys.org. (TTY users call 711, then follow the prompts to dial 844-614-8800).

Centers Plan for FIDA Care Complete wants to make sure you have access to plan materials in your preferred language. So, when you call, we’ll ask you for your preferred reading language and whether or not you want to get some of your materials in that language.  We might also reach out to you once or more a year to make sure the information we have on file about your preference is correct.  Of course, you are always able to make changes to your preference by:

  • Speaking with a live representative at 1-800-466-2745 (TTY/TDD users call: 1-800-421-1220 or 711), seven days a week, from 8am to 8pm;
  • Sending a letter to us at: Centers Plan for FIDA Care Complete, Attention: Participant Services, 75 Vanderbilt Avenue, Staten Island, NY 10304; or
