  • What is FIDA?

    FIDA is a demonstration project that introduces a model of care, which integrates and coordinates Participant benefits and care across the Medicare and Medicaid programs.   • What is Centers Plan for FIDA Care Complete?

    Centers Plan for FIDA Care Complete (Medicare-Medicaid Plan) is a managed care plan that contracts with both Medicare and the New York State Department of Health (Medicaid) to provide benefits of both programs to Participants through the Fully Integrated Duals Advantage (FIDA) demonstration.
  • What is Centers Plan for FIDA Care Complete’s Service Area?

    Centers Plan for FIDA Care Complete serves the following 5 counties in New York State:
    • Bronx
    • Kings (Brooklyn)
    • New York (Manhattan)
    • Queens
    • Richmond (Staten Island)
  • Who is eligible for Centers Plan for FIDA Care Complete?

    Residents of the 5 counties listed above, who are: 
    • 21 years old, or older, at the time of their enrollment:
    • Entitled to benefits under Medicare Part A;
    • Enrolled under Medicare Parts B and D;
    • Receiving full Medicaid benefits; AND who is also one of the following:
      • A Nursing Facility Clinically Eligible individual who is receiving facility-based long-term supports (LTSS);
      • An individual who is eligible for the Nursing Home Transition & Diversion (NHTD) 1915 (c) waiver; or
      • An individual who requires community-based long-term care services for more than 120 days, as determined by a State approved clinical assessment.

    New York Medicaid Choice (NYMC) is New York State’s Enrollment Broker for the FIDA program. NYMC provides free counseling about your FIDA Plan options and can help you enroll in or disenroll from a FIDA plan. New York Medicaid Choice is not connected with an insurance company, managed care plan, or FIDA Plan.

    To enroll in Centers Plan for FIDA Care Complete please call New York Medicaid Choice (NYMC) at 1-855-600-FIDA, Monday through Friday, 8:30 am to 8:00 pm, Saturday 10:00 am to 6:00 pm. TTY users please call 1-888-329-1541. This call is free.  You can also visit the NYMC website at www.nymedicaidchoice.com
