Welcome to Centers Plan for Medicaid Advantage Plus (HMO-SNP)!
Centers Plan for Medicaid Advantage Plus brings your Medicare benefits, Medicaid benefits, and long-term care services together under one plan. This plan was designed for people with Medicare and full Medicaid who need, or are expected to need, at least 120 days of community-based long-term care services. Centers Plan for Medicaid Advantage Plus allows you to remain in your home and community, receiving the services you need to maintain your health and safety.
As a member, you will be assigned a Nurse Care Manager who is a licensed professional who will work with you to help you:

- Manage your medical and long-term care needs;
- Work with your primary care physician to obtain needed services:
- Identify your goals;
- Create a plan of care to achieve those goals; and
- Help coordinate all your health services.

Centers Plan for Healthy Living’s (CPHL’s) Utilization Management Team (UM) focuses on the premise that quality medical care is the single-most-important element in delivering health care services. Our UM program is a coordinated and comprehensive program designed to monitor access to health care services to members. CPHL’s Utilization Management decisions are based solely on the medical necessity and appropriateness of care and service. Medical necessity means health care and services that are necessary to prevent, diagnose, manage or treat conditions in the person that cause acute suffering, endanger life, result in illness or infirmity, interfere with such person's capacity for normal activity, or threaten some significant handicap.
