At Centers Plan for Healthy Living (CPHL) our goal is to provide our Members with helpful information that empowers them to better manage their health. It is our belief that the more you know, the better you can manage the everyday challenges you encounter with your chronic condition(s). We want to help you and your family stay healthy and active.

Our team recognizes the central role YOU play in managing your daily health needs. We provide all the assistance you require to support you in this endeavor.
To help you manage your chronic condition(s) during times of challenge, our experienced care management team offers telephonic support and guidance as you transition between emergency or hospital care, your primary care office, and your home. We offer you support and coaching as you care for yourself at home.
CPHL provides a connection to Registered Nurses, Social Workers and a Certified Diabetes Educator. CPHL can offer guidance on health topics such as Diabetes, Congestive Heart Failure, COPD and Asthma.
CPHL provides information to help you stay fit, manage your disease, maintain your weight, eat healthy, and much more. We can help you to get access to the tools needed to manage your illness; and we can help you find classes, counseling and support groups for your needs.
One of our goals, here at CPHL, is to helping you develop specific self-management skills, improve your chronic condition(s), and optimize your overall health status.
Preventive Health Guidelines
We want you and your family to stay healthy. Regular checkups, even when you are not sick, are important to maintain your health. They also help your doctor identify health problems early on.
