Our medical personal and therapy staff consult with you, your family, your healthcare provider—and hospital if you have just been discharged—to create a path to recovery designed specifically for you and the challenges you face. That is the vital first step in your individualized Steps to Home plan of action.

Step number two is teamwork.

As the saying goes, teamwork makes the dream work. Your dream is to make a full recovery, to get back on your feet and back home. Our team is here to make your dream a reality. Depending on the nature of your condition and your treatment plan, you’ll work with our expert Physical, Occupational, and Speech Therapists. You’ll benefit from the most advanced rehab technology, such as BlazePod—an interactive training system that will help you improve balance, coordination, and mobility. It’s both effective and fun.

Granville Center: Home Should Not Be a Last-Day Conversation

At Granville Center on Madison Street, the “Steps to Home” idea has to start before anyone is talking about discharge papers. A patient may be doing fine in one therapy session and then struggle later with fatigue, balance, pain, or getting dressed without help. That is the kind of thing a family wants known before the ride home is arranged. Granville lists short-term care, long-term care, individualized rehab plans, rehab technology, nurses, therapists, onsite medical staff, and EMR/charting used across the care team.
