Using Technology to Keep Residents Safer and Healthier

At Centers Health Care, we are always looking for ways to enhance the care and comfort of our residents. One of the most meaningful steps we’ve taken recently is introducing the Circadia Intelligence Platform—an AI-driven predictive analytics and workflow automation platform to deliver insights and streamline clinical operations—that adds an extra layer of support for both residents and staff.

Our caregivers remain at the heart of everything we do, always attuned to the small changes that can make a big difference. Now, with technology by their side, they’re able to see even more.

As part of this effort, we’re rolling out the Circadia C200 System, an FDA-cleared, contactless, radar-powered sensor that continuously tracks respiratory rate, heart rate, and motion to help detect potential complications early. Integrated with the Circadia Intelligence Platform, it delivers continuous physiological data to help identify early signs of patient deterioration, enabling our onsite staff to proactively address clinical decline or fall risk.

We’ve already seen how this technology helps us stay ahead of emerging issues. It provides early alerts if a resident’s breathing patterns or activity indicate a potential illness, fall risk, or other health concern—often before symptoms become outwardly visible. This gives our nursing team valuable time to assess, intervene, and, when appropriate, treat in place, helping to prevent unnecessary hospitalizations and keeping residents comfortable in their care setting.

At Triboro Center in Bronx, New York, for example, these interventions have resulted in a 20-30 percent reduction in hospitalization rates since they were implemented in September 2024.

“Circadia has been a wonderful, additional watchful eye for our team,” said the facility’s administrator, Shragi Goldberg. “The real-time tracking gives us actionable insights we wouldn’t otherwise have and helps us provide even better care for our residents.”

To get the most out of the tool, Shragi has introduced a weekly team meeting where the interdisciplinary care team reviews Circadia’s data, shares insights, and plans appropriate interventions.

“The technology fosters better communication between our physicians and nursing staff. It gives us greater confidence knowing that Circadia is monitoring residents even when we’re not in the room. Families also value the added peace of mind it provides.”

The success of Circadia at Triboro Center and other facilities has inspired us to expand this program to more facilities across our network. We’re excited to bring this technology to additional locations in the coming months, furthering our commitment to proactive, high-quality care.
