Mr. Toro had been hospitalized with serious complications from type 2 diabetes—at risk for high blood pressure, heart disease, and kidney failure. The hospital put him on medication, but when he was admitted to Rochester Center, he was extremely weak, unable to dress or bathe on his own.
The Rochester Center team designed a holistic, individualized rehab plan for Mr. Toro. Physical and occupational therapy, specific changes in diet, and a regular schedule of recreational activities gradually restored his strength. Daily calibrations of his progress enabled the team to adjust his program in real time. The ultimate goal: restore his independence.
