  • Increase vigilance through radar-powered sensors that continuously monitor respiratory rate, heart rate, and motion.
  • Identify early physiological indicators of patient deterioration.
  • Mobilize onsite staff to proactively intervene in cases of clinical decline or elevated fall risk.

In the months following implementation, the technology has driven measurable improvements in care delivery and operational workflow. Results include:

  • A 20%–30% reduction in rehospitalization rates.
  • Weekly interdisciplinary team meetings to review predictive data and coordinate targeted interventions.
  • A streamlined approach to actionable insights that enhances clinical oversight without adding to staff workload—an important advancement amid ongoing workforce challenges.

The success of this initiative has been recognized by Skilled Nursing News, which honored Centers Health Care as a leader in clinical innovation. The publication cited the organization’s measurable improvements and its shift from retrospective care models to proactive, predictive approaches—where data informs timely, targeted action.
