From Hospital to Home: How Social Workers Power Seamless Care Transitions

On that note, this year, person-centered care is more important than ever. As a key part of our care team, our social workers and our relationships with hospital social workers are core to our success in this area.
This month is National Social Work Month, and as I wrote in Modern Healthcare, the strong collaboration between our social workers and their hospital counterparts is foundational to both a patient’s transitions and their ongoing care journey with us.
It’s an ideal time to acknowledge the dedication, resilience, and impact of our social workers, and our social work partners. Together with our clinical staff, they ensure that our patients and residents are supported at every step. That’s why we’re celebrating our colleagues and partners throughout the month of March. We’re grateful for every person who helps us deliver care that puts people first.
